Problems began when a fire started in a rear lavatory. Prior to the forced landing many of the passengers had already died of carbon monoxide poisoning and smoke inhalation. A possible cause of the fire was that the lavatory waste bin contents caught fire after a lit cigarette was thrown into it. There were 11 survivors.

A flat tire during take-off resulted in over-heating and structural weakening of the No. 1 tyre. The inflight fire consumed the cabin floor and bodies fell from the melting aircraft that finally became uncontrollable and crashed.
