All the patients, mostly depressed young women, were severely sedated and locked up behind barred windows. 30 patients were heavily burned. The fire started when a nurse, who also died in the disaster, helped escape a patient who set fire to solitary confinement room 22 and forgot to close the door.

The likely cause of the sinking was portholes that were opened because of the lack of air conditioning on the vessel, which allowed water to enter Bulgaria when the captain attempted to turn the ship during stormy weather.
