An electrical anomaly caused a Zuni rocket on a F-4B Phantom to fire, striking an external fuel tank of an A-4 Skyhawk. The flammable jet fuel spilled across the flight deck, ignited and triggered a chain-reaction of explosions that killed 134 sailors and injured 161.

Monday 25th of July 2016 (x) — Mass murderer Satoshi Uematsu stabs 19 mentally disabled people from 18 to 70 years old to death in a residential care center.
Another 26 patients were injured. Uematsu turned himself in at the Tsukui Police Station two hours after the incident with a bag of multiple kitchen knives and other bloodstained sharp tools.
